What You Will Get:

1. Discounted visits: Members enjoy a 10% discount for up to four (4) 60 minute appointments which can be broken up into 30 minute sessions as needed. These visits are transferrable to anyone you choose if you don’t need all four hours. They can be gifted to another person or even traded for services or used as vouchers.
2. Discount on upcoming Whole Human Tribe Membership! Dr. Windham will hold monthly to bi-monthly group calls to help you in your dis-ease to health journey and VIP members will receive 50% off their first year!
3. Discounted supplements: Members enjoy a 10% discount on all supplements bought through Doctors Supplement Store (DSS).
4. Discounted testing pricing: Members receive a 10% discount on applicable specialty testing.
5. Direct phone access to Physician: Physician will be available by text or phone call on a Twenty-Four hour per day, seven days per week basis to address any non-emergency issues you may have. You will be provided with Dr. Windham’s cell phone number that provides access upon signing the agreement.
1. Response times: Texts and voice messages left during the day will be responded to within 2 hours and within minutes if possible.
2. After hours: If Dr. Windham is available for an after hours call, she will pick up directly. Otherwise, voice messages or texts sent after 7 PM CST / 5 PM PST will be responded to that evening or the next day.
3. In the event of an emergency: Always contact 911 first in the event of a potential emergency and only then, if possible, the Member (or his/her designee) may contact the Physician.
4. E-Mail Availability.  Subject to the conditions set forth in this Agreement you will have e-mail access directly to the Physician and Practice for non-acute and non-emergent inquires.
6. Preferred Appointment Scheduling:
1. Admin Days: Dr. Windham takes one admin day weekly and doesn’t allow any appointments these days. However, Members can get appointments on admin days if their situation requires it.
2. Calls or e-mails received by the office prior to noon: Every reasonable effort will be made to schedule an appointment with the Physician on the same day.
3. Calls or e-mails received by the office after noon:  Every reasonable effort will be made to schedule an appointment with the Physician on the following normal office day.
4. Weekend appointments: On rare occasions, if Dr. Windham agrees that it is reasonable, weekend appointments can be arranged directly with the Physician.
5. Call the office to schedule your appointments with your 10% discount.
7. Limited patient enrollment to assure Physician access. In order to ensure personalization of care, the practice size will be limited to no more than 50 patients.
